Scientific background
When comparing a test, ask which population was studied and what reference method was used. Results from a specialist centre may not apply to routine screening.
A methodological study of 184 studies covering 218 diagnostic tests found that participant selection matters. Comparing separately selected cases and controls produced a diagnostic odds ratio three times as high as in clinical populations (95% confidence interval 2.0–4.5). This is a comparison of study designs, not a threefold increase in any particular test’s sensitivity.
TİTCK lists separate Turkish regulations for medical devices and in vitro diagnostics. For a performance evaluation, specify the intended use, target population and reference standard first. That brief helps identify a suitable evaluation team and gives the team the context needed to interpret reported performance.
